As recently as 1989, the U.S. Federal Railroad Administration (FRA) and the National Transportation Safety Board (NTSB) investigated a crown sheet explosion on a tourist railroad in Texas. This prompted FRA to tighten already-stringent regulations aimed at training firemen to properly administer feedwater to their crown sheets. Fortunately, the locomotive's crew, both volunteers, survived the accident.
